It includes a structured evaluation table and space for additional comments or recommendations. All committee members must sign the form, which must then be returned to the DSLS coordinator.
The TAC Evaluation Summary Report is a duplicate of the meeting report form, used to document the same structured assessment of the PhD candidate during TAC meetings. It ensures transparent progress tracking and agreement among committee members regarding the student’s development and project status.
